Clinicians provide clinical care and case management to transition age youth served through the Community Counseling, Esser House, Real Alternatives (RA), Next Steps Collaborative (NSC), NSC21, Aftercare, ReFocus and Our Space programs. The Clinician is part of a multidisciplinary team that provides community based mental health services and collaborates with the youth, housing, and other service providers in order to assist in their transition to adulthood. The Clinician will serve at-risk LGBTQ youth, transition-age youth exiting foster care and juvenile justice systems.

The Clinician will carry a caseload that averages 13-16 youth. This exempt position may require work weeks in excess of 40 hours as well as evening and weekend work.

Responsibilities

  • Participate in intake and discharge process.
  • Partner with youth in completing strength-based needs assessments related to the various transition domains.
  • Meet with youth weekly or more, to partner with them in identifying and working on short and long term mental health and independent living skills goals.
  • Partner with youth in promoting the involvement of family and other informal and formal key players in planning services and supports for each youth. Partner with youth in maintaining relationships with permanent support people and identifying opportunities to strengthen these relationships.
  • Coordinate and partner with housing program collaterals to achieve treatment and independent living goals.
  • Ensure culturally sensitive and relevant treatment.
  • Responsible for writing clinical reports (Progress Notes, Assessments, Treatment Plans and Discharge Summaries.) within expected agency timeframes.
  • Partner with youth in acquiring access to community settings and resources including but not limited to health care, employment, housing, social security benefits, etc.
  • Obtain and complete necessary clinical records/documents within assigned time frames, perform Medi-Cal administrative activities

Qualifications

  • Masters in Social Work or Marriage and Family Therapy or Professional Clinical Counseling  from an accredited university or college.
  • Registered (AMFT, APCC or ACSW) with California BBS.
  • LPC (Licensced Professional Counselor)
  • At least one year post-graduate experience working with adolescents, preferably current and/or former foster youth.
  • Community Care Licensing fingerprint clearance, which includes: California Department of Justice, FBI and CA Child Abuse Index.
  • A valid California driver's license, dependable personal vehicle for use on the job, insurable by SHS’s insurance policy based on DMV record, and proof of personal vehicle insurance.
  • Knowledge of foster youth, child development, domestic violence and substance abuse
  • Proficiency in Microsoft Office programs, particularly Word and Excel
