Youth Specialist - Transitional Housing Program

The Youth Specialist provides case management services to transition-age foster youth participating in the Transitional Housing Placement Program (THPP) and Transitional Housing Program Plus Foster Care (THP+ FC).  The THPP and THP+FC provide structured, supportive independent living for qualified youth some of whom have a child or are pregnant, and/or are have higher needs.  The goal of the programs is to provide a safe environment so that youth may practice the skills necessary to be self-sufficient. Youth live independently in apartments with a roommate and regular assistance provided by a support team. The Youth Specialist will carry a shared caseload of 10-13 youth with the Clinical Case Manager.

RESPONSIBILITIES:

  • Meet with youth weekly to partner with them in identifying and working on short and long-term independent living skills, employment, and educational goals.
  • Partner with youth in improving independent living skills (cooking, budgeting, shopping, cleaning, hygiene, laundry, maintenance, etc.), managing time, and balancing responsibilities
  • Use a strengths-based approach to partner with youth in creating a Transition to Independent Living Plan and developing and monitoring the progress of monthly goals.
  • Responsible for documentation of meetings in Electronic Health Records.
  • May be responsible for writing clinical Progress Notes in accordance with Medi-Cal guidelines and within expected agency timeframes.
  • Partner with youth in identifying community resources.
  • Partner with youth in identifying obstacles to living independently, obtaining employment, obtaining post-secondary education, and developing supportive social and recreational activities. Partner with youth to develop strategies to overcome these barriers.
  • Partner with youth to develop and monitor working goals and objectives with measurable outcomes.
  • Partner with parenting and pregnant youth in accessing resources to help them to become a successful parent, including but not limited to parenting classes, child development classes, etc.
  • Ensure culturally sensitive and relevant treatment
  • Complete monthly budget and collect rent and utility payments.
  • Serve as a community liaison with court-appointed advocates, medical providers, and legal representatives.
  • Plan and facilitate one monthly group
  • Provide support and monitor youth during times of crisis.
  • Support youth in moving in and moving out of program, including but not limited to packing up youth’s belongings and physically moving from one location to another
  • Complete monthly reports for tracking and statistical purposes.
  • Complete Transition to Independent Living Assessments semiannually.
  • Regular on-call responsibilities.
  • Transport youth in vehicles on a daily basis
  • Support Housing Services Manager in coordination, identification, and acquisition of housing stock to meet programmatic needs.
  • Serve as the program liaison with landlords and/or property management staff.
  • Receive and respond to property management concerns of program participant tenants. Work with program staff to resolve conflicts between program participants/tenants and landlords.
  • Understand and ensure compliance with California landlord/tenant law and/or the Transitional Housing Misconduct Act.
  • Conduct regular checks of all BAYC’s transitional housing apartment units and report any concerns to program staff.

QUALIFICATIONS:

  • BA in Social Science field
  • Experience working with transition-age youth, preferably current and/or former foster youth
  • Fulfillment of TB clearance, fingerprinting and state clearance, and any other mandatory state/federal requirements.
  • A valid California driver's license, dependable personal automobile for use on the job, insurable by SHS’ insurance policy based on DMV record and personal vehicle insurance.
  • Knowledge of the foster care system, youth development/youth-adult partnership framework,  the cycle of Domestic Violence, Parenting skills and bonding techniques, youth-friendly community resources and public welfare system
  • Proficiency in Microsoft Office programs, particularly Word and Excel
  • Commitment to self-reflection and personal awareness as a means of being able to remain emotionally available and connected with youth, and function as an engaged team-member
  • Passion for working with a diverse group of Transition Age Youth (TAY) including youth of color, LGBTQ youth, and youth with disabilities.
